Step 2: NOTICE OF PRIVACY PRACTICES. You will be asked to sign a statement that you have read and understand this document.

Step 3: Complete a HEALTH INTAKE FORM. Bring this with you to your first appointment. Contact Massage for Body & Feet at cuzyouareworth@yahoo.com or 929-3636 with questions and for directions.

FOR INSURANCE BILLING ONLY

Step 4: If you would like to pursue billing your health insurance, note that most insurance plans are accepted. Check with Massage for Body & Feet to verify that your specific insurance may be used.  Then bring a completed INSURANCE INTAKE FORM and a copy of your insurance card to your visit.

Step 5: Bring the completed form "WHAT CAN YOU DO?" Insurance companies use this information to provide a before and after assessment of your ability to manage every day activities due to the medical reason you need massage therapy.
